Monkfield Dental Plans – Terms and Conditions

Monkfield Dental Plan Terms and Conditions:

1. Patients will start benefiting from the plan six months after signing up, provided they have paid all monthly fees up to that point. Failure to maintain payments may result in a delay or suspension of benefits.
2. Plan Description: These dental plans are pre-payment schemes to spread the cost of preventative dental treatment over a twelve-month period and to provide additional discounts and benefits to Plan members. Patients will be able to choose one plan from the options offered.
3. Payment Terms: Monthly payments for the specific dental plan chosen by the patient will be collected via direct debit on or around the 1st of each calendar month.
4. Cancellation Policy: The patient can withdraw from the plan at any time by providing a minimum of three months’ notice in writing. The dental practice reserves the right to withdraw a patient from the plan at its discretion at any moment by providing a written notice via email or post. Fees paid up to the point of withdrawal will not be refunded, and all benefits associated with the plan will cease immediately upon termination.
5. Missed Payments: In case of a missed payment, a direct debit will be attempted 7 days after the initial failed payment. If this second payment fails, the Dental plan and its benefits will be suspended until the outstanding balance is cleared.
6. Appointment Policy: Appointments can be booked by phone, in person at the dental practice, or online through our online booking system. Missed appointments or appointments cancelled with less than 24 hours of notice will incur a Fail to Attend fee, up to the full cost of the appointment. The patient’s plan may be suspended or cancelled until all outstanding Fail to Attend fees are paid. In the event of such suspension or cancellation, no refund of monthly fees paid by the patient will be granted.
7. Annual Entitlement: Unused appointments or services cannot be carried over to the following year.
8. Cooling-off Period: The patient can cancel the Plan and receive a full refund within 14 days of receiving the subscription confirmation.
9. Transferability: This agreement is not transferable by the patient.
10. Price Changes: Price changes in the dental plan will be communicated to patients via email with at least 1 month’s notice.
11. Data Protection: Patients’ personal information will be handled with the strictest confidentiality and in accordance with GDPR rules.
12. Services Covered: A clear outline of services covered under each plan is provided along with these terms and conditions in the same email.
13. Changes to Terms and Conditions: The dental practice reserves the right to update or modify the terms and conditions of the Dental Plan. Patients will be notified of any changes in advance.
14. Acceptance of Terms: By enrolling in one of the dental plans offered, patients explicitly accept and agree to abide by these terms and conditions in their entirety.
15. Enrollment Eligibility: Patients can only enroll in a dental plan if they meet all of the following conditions: a) The patient must be registered with the dental practice. b) The patient must have completed a dental check-up with the practice. c) The patient must be deemed dentally fit, having completed all required treatments advised during their most recent check-up.
